Update GD docs using the actual exported bindings as a base. Consolidate and
update existing GD and OD docs. Output HTML.

Motivation is:
a) the existing docs at http://wwwopendylan.org/gdref/gdlibs.html are
out-of-date in various ways;
b) they are also very incomplete; and
c) it's hard to find things in the existing docs.

See gd-hackers e-mail of 11 Feb 2007 for more info. Basically, the pages will
be generated from text files using a variation of NaturalDocs located at
http://homepage.mac.com/d_j_v/FileSharing5.html. The text files will be added
to SVN, the HTML files will generate automatically external to SVN.
